Woman arrested for insurance fraud

Huntingburg police arrested a woman after an insurance fraud investigation revealed she allegedly used a false identity while receiving medical treatment.

The incidents were recorded as far back as June of 2012.

The Huntingburg Police Department launched the investigation in October after receiving a report from an insurance company involving someone going by the name Sarah Ann Hernandez.

According to Detective Sgt. Christian Gogel, his investigation revealed that Berenice Barrientos-Galcana, 42, of E. Second Avenue, was hired by a local furniture company under the alias of Sarah Ann Hernandez. Barrientos-Galcana allegedly had a fraudulent date of birth and fake Indiana Driver’s License under the assumed name.

The insurance carrier became aware of the fraudulent identification after it was received from Memorial Hospital.

Tuesday evening, Gogel, with the assistance of Dubois County Sheriff’s Department deputy Jesus Monarrez, spoke with Barrientos-Galcana at the Huntingburg police station and subsequently arrested her for Level 6 felony synthetic identity deception.

Authorities booked Barrientos-Galcana into the Dubois County Security Center.
