Parents asked to take immunization survey

The Dubois County Health Department is working on a project to assess overall child immunization rates in Dubois County.

In order to do so, they are looking for parent’s feedback on their child’s immunization habits and services.

This survey is an anonymous five-question survey that can provide feedback to help the health department better assist the community’s needs. Take the survey here.
