John E. Forman, 68, Shoals
John E. Forman, 68, of Shoals, passed away surrounded by family at his home at 5:39 p.m. on Sunday, May 24, 2026.

John was born in West Virginia on July 20, 1957, to Oral and Geraldine Sarah (Barger) Forman.
He has worked for various employers over the years, most recently at the French Lick Springs Hotel. Before that, he worked at the West Baden Springs Hotel for eight years. He was a very kind-hearted person and always willing to help others in need. Throughout his life, he was a caregiver for various family members, primarily his older sister, Gerry Ann Kalb.
He deeply valued his Catholic faith. He enjoyed riding bikes, going on picnics, being outdoors in nature, collecting items, playing games, learning about his family’s genealogy, and watching television. He enjoyed spending time with his family, especially his older sister, Gerry Ann and their dog Frisbee.
John is survived by his sisters, Gerry Ann Kalb of Shoals and Carol Forman of Lafollette, Tenn.; one brother, Daniel Forman of Bradford, Ohio and a half-brother, William Keffer of West Virginia; along with several nieces and nephews.
Preceding him in death, besides his parents, were two brothers, David Forman and Roger Forman, and an infant sister, Jerinda Forman.
Visitation will be held from 4:00 to 7:00 p.m. on Monday, June 1, 2026, at the Becher-Kluesner North Chapel in Jasper, Indiana. Fr. Keith Hart of St. Joseph Catholic Church will officiate a prayer service at 7:00 p.m. at the funeral home. Burial will take place at a later date at Willow View Cemetery in Dayton, Ohio.
