Covid-19 vaccinations for age 6 months and older available at Dubois County Health Department

The recent expansion of COVID-19 vaccine eligibility now allows anyone 6 months and older to be protected against the disease. 

Earlier this week, both Moderna and Pfizer vaccines received authorization from the FDA and Centers for Disease Control and Prevention (CDC) to allow use for any individual 6 months and older. The Dubois County Health Department will continue to carry both vaccines, offering individuals a choice for their primary series.  

If you are eligible to receive the Covid-19 vaccine, no one will be turned away. Patients are encouraged to bring some form of identification to their appointment. Insurance is not required; however, information will be collected if you have coverage. 

Starting Monday, June 27, 2022, you can visit www.ourshot.in.gov to schedule an appointment or call 812-481-7056 for assistance. Walk-ins will be accommodated to the best of our ability.

Reminders:  

  • Our drive-thru is only available to those 12 years of age and older. 
  • A parent needs to be present for anyone 6 months to 18-years old. If this is not an option, you MUST complete the CONSENT FOR COVID-19 VACCINATION FORM attached below. A copy of a parent’s ID and insurance card must be included if the patient has coverage.

La reciente expansión de la elegibilidad para le vacuna COVID-19 ahora permite que cualquier persona de 6 meses o más está protegida contra la enfermedad.

A principios de esta semana, las vacunas Moderna and Pfizer recibieron la autorización de la FDA y Los Centros para El Control y La Prevención de Enfermedades (CDC) para permitir su uso en cualquier persona de 6 meses o más. El Departamento de Salud del Condado de Dubois continuara ofreciendo ambas vacunas, ofreciendo a las personas una opción para su serie primaria. 

Si es elegible para recibir la vacuna Covid-19, nadie será rechazado.  Se le motiva a los pacientes traer algún tipo de identificación a su cita. No se requiere seguro; pero si, se recopilara su información si tiene cobertura. 

A partir del lunes 27 de junio de 2022 se puede visitar www.ourshot.in.gov para programar una cita o llame al 812-481-7056 para obtener ayuda. Las visitas sin cita previa se acomodarán lo mejor que podamos. 

En la página de Facebook del Departamento de Salud del Condado de Dubois se puede encontrar un calendario de vacunas conveniente contra el Covid-19. Haga clic en “me gusta” en Facebook para seguir futuras actualizaciones relacionadas con la salud a medida que ocurren.

Recordatorios: 

  • Nuestro autoservicio solo está disponible para mayores de 12 años. 
  • Un padre debe estar presente para cualquier persona 6 meses a 18 años. Si esta no es una opción, DEBE completar el FORMULARIO DE CONSENTIMIENTO PARA LA VACUNA CONTRA EL COVID-19 adjunto a continuación. Se debe incluir una copia de la identificación de los padres y la tarjeta del seguro si el paciente tiene cobertura.
